Please use this organizer to provide essential documents and details for accurate and timely preparation of your federal and state corporate tax returns.
1. Company Information
Please provide the following:
- Legal business name
- Federal Employer Identification Number (FEIN)
- California Secretary of State Entity Number
- Date of incorporation and state of incorporation
- S Corporation election effective date (if applicable)
- Business address, phone, and email
- Principal business activity
- Ownership structure and any changes during the year
- List of shareholders (include name, SSN, ownership %, and addresses)
- Copy of the prior year’s tax return is required to e-file
- If new to our firm, include the last 3 years of corporate tax returns
2. Financial Statements
Please provide your complete year-end financial statements, including:
- Profit and Loss Statement (Income Statement)
- Balance Sheet (Assets, Liabilities, and Equity)
- General Ledger (optional but helpful)
- Trial Balance
- Bank statements for year-end reconciliation
- Inventory reports (if applicable)
- Fixed asset list with acquisition dates and depreciation schedules
3. Income Documentation
Please provide details on:
- Total gross receipts or sales
- Returns and allowances
- 1099-NEC or 1099-MISC received for services
- Interest and dividend income (include 1099-INT and 1099-DIV)
- Gain or loss from sale of assets (include supporting documentation and Form 4797, if applicable)
- Rental or royalty income
- Other business income (e.g., grants, rebates, forgiven PPP loans, etc.)
4. Deductions and Expenses
Please provide a breakdown of the following business expenses:
- Wages and salaries paid to employees
- Payroll tax reports (Forms 941, DE9/DE9C, W-3 and W-2s)
- Contract labor (include 1099s issued)
- Rent or lease expenses (equipment, vehicles, real estate)
- Advertising and marketing
- Insurance (health, liability, business property, etc.)
- Office supplies and expenses
- Meals and entertainment
- Travel and vehicle expenses (include mileage logs if applicable)
- Professional fees (legal, accounting, consulting)
- Bank and merchant processing fees
- Utilities, internet, and phone
- Depreciation and amortization (include asset details if new purchases)
- Charitable contributions (if any)
- Other deductions (itemize if material)
5. State and Local Requirements
Please answer the following:
- Did the corporation operate only in California during the year?
- Any income or operations in other states?
- Estimated tax payments made to California (include dates and amounts)
- Any business licenses or gross receipts taxes paid?
- Did the business pay rent for a California location (for purposes of credits or apportionment)?
- Did the business participate in CalSavers or offer retirement benefits?
6. Other Important Information
Please indicate whether any of the following apply:
- Any ownership changes during the year
- Any distributions or shareholder loans made
- Any shareholder health insurance premiums paid by the corporation
- Any new EINs, DBAs, or address changes during the year
- Did the business receive a Form 1099-K from third-party payment platforms?
- Did the company use cryptocurrency for any business transactions?
- Were there any IRS or FTB correspondence, audits, or notices during the year?
